NEW CITY, NY — The Rockland County Department of Health (RCDOH) is inviting all residents to play a role in shaping the future of health in Rockland County by participating in the Community Health Assessment Survey. This brief, anonymous survey is designed to gather input on the health issues that matter most to you and your community. The survey only takes 10–15 minutes to complete and will help guide health priorities for the county through 2030.
The deadline to submit your survey is July 31, 2025. Your feedback is invaluable in helping the RCDOH and local partners—such as hospitals, nonprofits, and other community organizations—improve health services and outcomes for everyone in the county.
How to Participate:
- Online: Complete the survey in English, Creole, or Spanish by visiting the Community Health Assessment Survey page.
- In-Person: You can pick up and return your survey in English, Spanish, Creole, Yiddish, and Hebrew at one of these locations during regular business hours:
- Blauvelt Free Library – 541 Western Highway, Blauvelt, NY 10913
- Clarkstown Town Hall (adjacent to the Town Clerk’s office) – 10 Maple Avenue, New City, NY 10956
- Finkelstein Memorial Library – 24 Chestnut Street, Spring Valley, NY 10977
- Church of St. Joseph and St. Boniface – 245 North Main Street, Spring Valley, NY 10977
- Rockland County WIC – 14 South Main Street, Spring Valley, NY 10977
